Registered Nurse - Caregiver Support
James E. Van Zandt Veteran Health Care System provides acute medical, telemetry, behavioral health, physical medicine and rehabilitation, geriatric, Women's' Health and primary care services. Home Based Primary Care is provided in the Veteran's home. The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well- being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community. (ANA Code of Ethics for Nurses). Caregiver Support Program Nurse shall provide services to Veterans in the Caregiver Support Program. This population includes all ages of men and women. He/she will collaborate with other members of the CGS team in the screening and evaluation of the Veterans in the CGS Program, as well as serve as liaison and consultant to the inpatient, outpatient, and community agencies involved in the provision of care to the identified Veteran. The nurse will be responsible for the completion of the assessments for the Caregiver Support Program, to include assessing the Veteran's functional capacity, the caregiver ability to provide care, and assessments of the home environment through home visits. This Mental Health Service (MHS) nurse shall provide leadership to other members of the interdisciplinary treatment teams in ensuring comprehensive planning and services for the population served. The incumbent will utilize the nursing process to evaluate and lead the implementation of program improvement evidence-based changes. He/she may be responsible for the completion of outcome/performance measures as required by the Caregiver Support Program. He / she may also be responsible for the facilitation of supportive services to include individual and / or group sessions for caregivers. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. This position requires potentially long periods of continued walking, standing, stooping, sitting, kneeling, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or during patient procedures and care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Use of fingers, both hands, and near and far vision is required.
