Social Worker (Health Care for Homeless Veterans - Program Coordinator)

Duties include, but are not limited to:
Serves as the medical center's primary point of contact for the National HCHV and HUD-VASH Program, Office of Mental Health Services, and VA Central Office. The incumbent will attend monthly clinical calls provided by the HUD-VASH Office and are available in TMS and can be accessed for training purposes from the Operational Planning Hub. The Program Coordinator will utilize and assist in maintaining a data monitoring system to measure the efficacy of the program. The Social Worker assists in collecting and analyzing local data for reports for the Facility, VISN Homeless Coordinator, VISN Mental Health Lead, National HCHV and HUD-VASH Program, Office of Mental Health Services, and VA Central Office. She/he will assist in evaluating the programs' progress and outcomes and utilizes data to continuously modify and improve the program and processes. As shared responsibility with other staff members, the incumbent is responsible for the program compliance with the Joint Commission (TJC) and Commission on Accreditation of Rehabilitation Facilities (CARF) standards of care. The incumbent is responsible for independently planning and coordinating CHALENG and Summit meetings and Stand Down as prescribed by program requirements. Along with the HCHV Case Managers, the incumbent will maintain a caseload and provide a full range of social work services within commonly accepted standards of social work practice which includes casework, individual, family and group counseling, psychosocial treatment discharge planning, case management and other services as appropriate to homeless Veterans in the community, as well as provide advanced level psychosocial and/or case management. The incumbent provides consultation of many programs including orientation, training and development. They will also distribute caseloads with consideration to the CTI model of care and ensuring that Housing First principles are consistent amongst the HUD-VASH Case Management team. He/She will also support a multidisciplinary team model of care and ensure that case management members have a shared caseload, while assessing the overall acuity of the population and identifying specific needs of this population. The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following:
Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Previous experience working with the homeless population. Grade Determinations:
Social Worker (Program Coordinator), GS-12 1. Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. 2. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. 3. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. This position requires walking, kneeling, bending, standing, and light lifting. Much of the work occurs within a typical office setting, either individually or in meeting with others, or in meeting rooms. Requires time to be spent walking. The incumbent may be exposed to infectious diseases and medical/psychiatric emergencies. In carrying out responsibilities, operation of a motor vehicle with a valid driver's license is required. The incumbent must meet the requirements to use a government vehicle in accordance with VHA directive 2008-020. It may be necessary for the incumbent to travel throughout the Altoona VAMC catchment area where he/she conducts interviews with the Veterans, family members, representatives of community health, welfare and law enforcement agencies
