Job Description:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in providing exceptional personal care services.
The ideal candidate will play a crucial role in ensuring the well-being and comfort of our residents/clients while upholding the highest standards of care.
Responsibilities:
Deliver personalized care to residents based on individual needs and care plans Perform nursing assessments and assist in developing personalized care plans Administer medications and treatments as prescribed by healthcare professionals Monitor residents/clients' health status and promptly address any changes or concerns Collaborate with caregivers, families, and healthcare professionals to ensure comprehensive care Provide education and support to residents/clients and their families regarding health conditions and care plans Maintain accurate and updated documentation of care provided
Qualifications:
Valid LPN license in Pennsylvania Experience in personal care, assisted living, or a similar setting preferred Strong knowledge of nursing principles and practices related to personal care Compassionate and empathetic approach towards residents/clients Excellent communication and interpersonal skills Ability to work effectively in a team environment
